Supported Cluster Housing

Supported Cluster Housing is an intensive housing model for individuals 18+ that provides a transitional setting for individuals that grapple with multiple barriers to independent living.

This transitional model places priority on preparing individuals to live full autonomous lives by providing a place of safety and unconditional support. Each cluster unit has on-site staff, seven days a week, to help attend to individual basic support needs. Resources include: access to food and nutrition, social and emotional connection, physical and mental health resources, external system navigation, medication support, and timely access to staff that can assist with problem solving and attending to concerns as they arise.
