What is Personal Outcome Measures?

Personal Outcome Measures or POM is a planning tool that asks people who are supported what they would like their life to look like.  There are 21 “outcomes” or indicators that ask about important things in 5 broad categories- My Human Security, My Community, My Relationships, My Choices and My Goals.  Through an interview process both with the person receiving support and a staff person that knows the person well, the interviewer can learn where there are opportunities for growth and development for the person.

Personal Outcome Measures asks questions to people supported by New Directions that we often forget to ask through the routine course of everyday services.  Through these conversations, we learn more about people, help them gain education, experience and exposure in all areas of life.  All of the outcomes are equally important, and it is up to the person to decide what they want to work on. As a result of adding this resource to all New Directions’ Adult Services, we are collectively shifting our priority to ensure people are asked what they want, and we help people lead the lives they choose!
