Adult Home Share connects a person needing support with a Home Share provider and they live together in the provider’s home.
Support offered to people is unique and individualized based on the goals, needs and wants of the person. A case manager works with every person to develop and implement person centred plans with the person, their supports and other services. Case managers also work with Home Share providers to understand needs of people. We encourage people to make decisions for themselves using support networks for guidance, and respect decisions even when they might not be what we would choose.
While Adult Home Share is primarily a residential service we look at the whole person and help the person identify and articulate the things they need to lead a fulfilled life. This may include different housing options, recreational opportunities with paid staff, volunteer or employment options, exploration of community places and expanding friendships and relationships. We believe all people have strengths and gifts, and we are all searching for our “place”, not just placement.
